CONTINUOUS QUALITY IMPROVEMENT TEAM (CQI)
Annual Report – July 2004
The CQI Committee started off the
new fiscal year with Theresa Thompson, Paul Bains, and Colleen Mudry
as your peer record reviewers.
North Shore Outreach, Tri-cities and
Ridge Meadows had a file pull. All these programs were commended on
their resilience with regard to changes in service delivery and
their work within the community they served. Based on these file
pulls, your CQI committee discussed: the log format; follow up
procedures; internal transfer of files; feedback forms; goal forms;
and how all of these could be improved to follow the “best practice”
guidelines.
Bernadine heats, as the leader, had
a number of topics and concerns to be presented to Management. Paul
Bains stepped into the position as leader when Bernadine was no
longer able to commit to the CQI team. Paul will take topics and
concerns to a Management Meeting as soon as possible.
Co-coach, Sharon Sargent, helped the
committee stay focused on eh mandate of the CQI team, namely, to
look for notable trends, and make sure each program is focused on
the measured outcomes model. Sharon also discussed re-accreditation!
Yes, folks, the 7th Edition is being worked on as we
speak, and your CQI team will figure out how this will affect the
team and will report at a later date.
Jacqueline Jones, the facilitator,
made sure the committee stayed on topic, on time, and to the point.
Jacqueline has now become a peer record review so we are in need of
a new facilitator. Time required is attendance at three quarterly
meetings plus an hour to do required reports.
As the Scribe, Anne Stiphout made
sure the minutes of the meetings were done. There were three
meetings this fiscal year 2003-04. Anne was kind enough to step in
to do file pulls when no one else was available. Thanks, Anne! Anne
would appreciate someone taking over the job of scribe as her time
is up . . . please folks, give it some serious thought.
Once again, your CQI team is
appealing for members to join the ranks! Colleen Mudry and Bernadine
Hearts are no longer with the Agency, so we need 2 new peer record
reviewers.
Theresa Thompson has resigned from
the committee; we still need 2 peer record reviewers!
As we are still being accredited and
the CQI Team is an important part of this pro0cess, we encourage
staff participation as this is an excellent committee to be a part
of, as it allows staff the opportunity to work with people from
other teams as well as visiting other program sites.
